Equipment Request Form

In order to fill out form, your PC must have Microsoft Word installed. When asked, select to open the form. Fill it out, then click "File", "Save As...", pick a place to put it, name the file something distinctive, then click the "Save" button. Next open your email program (example... Outlook Express) and send the form that you just saved as an attachment to this email address- request@4bmc.com
